Question: Doctor, my son has been diagnosed with type 1 diabetes. I have seen one online advertisement on diabetes treatment and contacted them. They told me, they can help in diabetes reversal and asked me to subscribe their Rs5000 package. Shall I subscribe to that Rs5000 package? kindly guide me.

Answer:  1. Please do not subscribe to any diabetes management package by an online diabetes clinic. Most of these clinics are run by non-qualified doctors. All type 1 diabetics should be strictly managed by offline direct consultation and preferably by MD medicine/ endocrinology/pediatrics doctors. 2. Type 1 diabetes is caused by the destruction of insulin-producing cells. Due to the absence of insulin-producing cells type 1 diabetes can not be reversed by natural means. So if someone is saying they can reverse diabetes naturally and asking you to pay Rs5000,  don't believe in them.  3. Type 1 diabetes can be easily managed by insulin. Question of the day: Doctor my recent blood investigations has shown elevated blood glucose. Where should I take my treatment?

  Answer: First/ initial diabetic consultation should be always offline i.e. at doctor's clinic (preferably doctor post-graduated in Medicine/ Endocrinology/ Paediatrics. Physical examination is very important in diagnosing the type and subtype of diabetes which is not possible in the online consultation. Once the type/subtype of diabetes is diagnosed, the doctor can decide on diet/ activity and drug intervention. We NEVER recommend online initial management of diabetes. Answer: Fasting plasma glucose ≥126 mg/dL (7.0 mmol/L) or HbA1c≥6.5%, or 2-h plasma glucose ≥200 mg/dL (11.1 mmol/L) during an oral glucose tolerance test, or a random plasma glucose ≥200 mg/dL (11.1 mmol/L) with classic symptoms. If a patient meets the criterion for HbA1c (two results ≥6.5%) but not FPG (<126 mg/dL or 7.0 mmol/L), or vice versa, that person should still be considered to have diabetes.
